Privacy Policy Notice

PURPOSE OF THIS NOTICE

Title V of the Gramm-Leach-Bliley Act (GLBA) generally prohibits any financial institution, directly or through its affiliates, from sharing nonpublic personal information about you with a non-affiliated third party unless the institution provides you with a notice of its privacy policies and practices, such as the type of information that it collects about you and the categories of persons or entities to whom it may be disclosed. In compliance with the GLBA, we are providing you with this document, which notifies you of the privacy policies and practices of Fetterman, Millinghausen & McNutt, Inc.

The GLBA further requires that we inform you that you have a right to prevent us from sharing nonpublic personal information about you with a non-affiliated third party for any purpose that is not specifically authorized by law. Your right to prevent us from sharing nonpublic personal information about you with a non-affiliated third party for a purpose that is not specifically authorized by law is called your right to "opt out" of such information sharing.

PRIVACY STATEMENT

Your privacy is a priority for Fetterman, Millinghausen & McNutt, Inc. and its insurance affiliates. We value the relationship we have with our policyholders and understand that you trust us with personal information about yourself, your belongings and your family. We are committed to ensuring that your personal information is used in an appropriate manner. We do not sell your personal information to anyone and do not share your customer information with others, except as permitted by law.

This Privacy Statement applies to our current and former customers who obtain our personal, family or household insurance products, as well as claimants and beneficiaries of those products. Personal information means information that identifies you personally and does not include information that may be obtained from a public source. We promise to protect your personal information by following the procedures described in this Privacy Statement. Please read the following information to learn more about how we collect, share and protect your personal information.

1) Information we collect
We collect personal information about you to complete transactions you initiate, service and manage your account, and inform you about our products and services. We collect nonpublic personal information about you from the following sources:

• Information you provide on applications, other forms, or by phone. This may include information such as your name, address, date of birth, and social security number.

• Information regarding transactions with us, such as your payment history, policy coverage, and claims history.

• Information we receive from consumer reporting agencies and other outside sources, such as motor vehicle reports, claims history reports, credit score or credit history reports and lending institutions.

2) How We Use Your Personal Information

We do not share your personal information with anyone except as permitted by law. We do not sell your personal information to third parties. We do not give your personal information to third parties for any type of promotional or marketing purposes not related to the sales or service of insurance products offered to you by our insurance agency.

We may share your personal information related to your transactions and experience with us with our insurance affiliates to better market and serve your insurance needs.


• We may also share your personal information with parties other than our insurance affiliates, such as an automobile dealer or lending institutions with whom you have contracted or claims adjusting service companies to assist in settling your claims.

3) Information we may disclose to third parties

We may disclose the information we collect, as described above, about our customers or former customers, to companies that perform marketing services on our behalf or to other financial institutions (Insurance Companies) with whom we have joint marketing agreements. We also may disclose information about our customers or former customers as permitted by law.

4) Non-affiliated third parties to whom disclosures may be made

A. Non-affiliated Third Parties To Whom Disclosures May Be Made

We may disclose nonpublic personal information about you, such as we have described above, to the following types of third parties that perform marketing services on our behalf or with whom we have joint marketing agreements:

• Fulfillment service providers, such as envelope stuffing services, billing and enrollment services.

• Other insurance agencies or companies that perform services for us in connection with carrying out an insurance transaction involving you or your policy.

• Financial institutions with whom we have joint marketing agreements, such as insurance providers.

• Other types of financial institutions such as premium finance companies.

We may also disclose nonpublic personal information about you to non¬affiliated third parties as permitted by law.

B. Notification of Your Right To Opt Out of Certain Disclosures

As we have indicated in this Privacy Policy Notice, we collect certain nonpublic personal information about you, and we may disclose that information to certain non-affiliated third parties for purposes other than those expressly permitted by the Gramm-Leach-Bliley Act and the federal and state regulations implementing that Act. If you prefer that we do not disclose nonpublic personal information about you to non-affiliated third parties, you may opt out of those disclosures, that is, you may direct us not to make those disclosures (other than those disclosures that are expressly permitted by the Gramm-Leach-Bliley Act and its implementing regulations).

If you wish to opt out of such disclosures to non-affiliated third parties, please so indicate on the attached Opt Out Form, which follows.

5) Our practices regarding information confidentiality and security

We restrict access to nonpublic personal information about you to those employees who need to know that information in order to provide products or services to you. We maintain physical, electronic, and organizational safeguards to protect information about you. We hold our employees responsible for upholding our privacy standards.

Please read the text below and decide whether you wish to exercise your right to opt out of the information sharing described. If you choose to exercise your right to opt out, you must return this form to us at;

If you return this form to us by mail, your response must be postmarked no later than 30 days from the date you received this notice from us in order for it to be valid. If you do not return this form to us within 30 days, you have not exercised your opt out right, and we can share the information described.

I wish to exercise my right under the Gramm-Leach-Bliley Act to opt out of Fetterman, Millinghausen & McNutt, Inc. sharing nonpublic personal information about me to non-affiliated third parties for purposes other than those that are permitted by law.
